You'll need to follow the instructions for the application for each school and each specific department you're applying to. You probably will have to have a TOEFL and GRE score, you'll probably have to submit your transcripts from all post-secondary work, and you will at least need to submit a personal statement.

On the web page for each university, you'll want to find a link called something like "Prospective Students" and click on that. Then you'll want to find information about applying as a graduate student. That's where you'll probably find the information you're after.

If you are hoping to attend a school where you'll write a dissertation supervised by a faculty member at that school and advised by a committee, it wouldn't hurt to contact faculty who have similar research interests to you and ask about opportunities that might be available to you. If, on the other hand, you're going to medical school or something, you won't need a mentor.
