They may send the form out to you, but you can also download it for yourself from http://www.direct.gov.uk/en/EducationAndLearning/UniversityAndHigherEducation/StudentFinance/DG_171624Depending on your parents circumstances, there are a few forms to choose from covering benefits, self-employment or regular income.
As for the Maintenance Grant, that is what you are applying for by filling the online form out, and you only choose to apply for the loans by filling in a certain part of the form. After they assess your application, you will get a letter through telling you how much grant/loan you are entitled to. You do not pay the grant back, but will have to pay the loans back after graduating and earning more than £15000pa.
